DISTAR Info Needed

I have a teacher taking a graduate course has been assigned to report on DISTAR
(Direct Instruction Systems in Arithmetic and Reading), and CPR/DISTAR (a
similar method for older youth). He was warned that there may not be much
information through the normal channels (which has proven correct), although
the professor thought that DISTAR was still used in Texas (? TENET).

Any information will be appreciated. My teacher is also looking for a copy of
the DISTAR assessment instrument for his presentation on the 30th.
