Target: Copyright Waiver

Last year someone had shared a copy of a Copyright waiver that they had
their faculty members sign acknowledging that they had read and understood
the copyright guidelines as part of the faculty handbook.  Does anyone have
something similar that they can share with those of us who have worked all
summer and are more or less braindead.
