Help with poem

Hi -

We are looking for a poem about "the last leaf on the tree". It's about
an old person reflecting on the fact that he/she has outlived all of
his/her friends and is awaiting death - like the last leaf on the tree
waiting to fall off. Does this ring a bell with anyone???
