REF: Need book title/author

Help!  One of my teachers saw a book at a workshop, can't remember what it was
called or who authored it, but wants it DESPERATELY!  It had something to do
with "falling through the cracks"  It is a picture book with lots of
illustrations that quite literally depicted school children falling through
cracks in the floor...actually addresses the sad fact that there are children in
schools who somehow don't get the education they should have.  Please email me
