Internet Driver's License

For those of you who issue Internet Driver's Licenses to students in
order that they be allowed to use the 'Net, on what basis are these issued?

Do the students complete a "course" of come kind, and/or must they
demonstrate soem specific competencies?

PLEASE, respond directly to me (in addition to the whole list if you
choose), as I am too snowed under at present and have temporarily stopped
