I used Classroom Connect's suggested form:
Author. Title of item. [Online] Available http://address/filename, date of
document or download.

But MLA and others might be different. MLA is this:
Author [if known]. "Title" (main title if applicable). Last date updated or
revised [if known]. <URL> (date accessed).
