There's a very nice one page evaluation form on page 105 in Sally
Mason-Robinson's (formerly ALA's Director of Video and Special Projects and
Publisher of _Media and Methods_) _Developing and Managing Video Collections:
A How-To-Do-It Manual for Librarians_.  She also has criteria on pages 103
and 104.  All of Chapter 2 deals with evaluation.
