TARGET: Book: How to write report card comments

I seem to recall seeing a book that was a "how to" for teachers on report
card comment writing.  I had a teacher ask for it and it would be useful
in my school because we have many teachers for whom English is a second
language.  Can anyone give me title/order information?  TIA!!!:-)
