HIT: suggestions for parent surveys

In response to my request for information about surveying my school
community, I received the following information.  I was not interested in a
library survey, just a general survey about our school.

The Souther Association of Colleges and Schools (an accrediting agency for
SE US) has survey forms.

The State of Missouri site  http://services.dese.state.mo.us    MSIP review
of schools

The Carnegie Foundation report on middle schools.    Try a search.

I've also collected some samples from schools in my district, Tulsa Public
Schools.
