internet evaluation form

Many of you asked me to post the internet site evaluation form I use with
my 7th graders.   The "Cyberhound" I refer to on page 2 is Gale's
Cyberhound's Directory of Internet Databases, which comes out yearly and
its title seems to vary a bit from year to year. I invite comments!
