TECH?: CD-ROM evaluation

        Tomorrow (Monday) I begin teaching a grad credit information tools
class for my staff. One of the areas I'm going to cover is CD-ROM
resources. Do any of you have a CD-ROM evaluation form/tool online, or
know where I can find one that I can print out and use with this class? Or
one that you can fax to me?  My fax number is 740-599-5863.  Since I just
survived a traumatic E-rate deadline, I haven't had time to reinvent the
wheel.
