I've used the following the 4 "R's" for years, with all grade levels.
We discuss them at every 'first class' of the year, they're
posted on the library wall (in VERY big print), and are also
in the Library Handbook:

READ -- not just books and other resources, but also the 'room'
-- e.g., what's happening in here?  What should I be doing?
RESPOND -- and be responsible for your actions. . . at all times.
RESPECT -- each other, library property, and even yourself.
RELAX -- the library is a non-violent zone.  Leave your anger outside.

After a while, all I have to do is look (with that inimitable teacher glare)
at a kid and ask "did you forget the 4 R's?"  and they get the point.

I also use 3 silly "alert" messages, as a polite way of reminding kids
to do something:
"Rug Alert" means push in your chair(s), so no one trips and
gets an awful rug-burn on their face when they fall.
"Slob Alert" means put away the materials you used:  your mother
isn't here to pick up after you.
"Turkey Alert" signals a truly dumb question (since domestic
turkeys are not known for their intelligence); as in, would you
like to think about what you just asked, and either figure it out
on you own, or ask a friend for help before you betray the fact
that you weren't paying attention to the directions given.

By mid-year, many kids will even give alert messages to each other
before I need to do it!
