GEN: LMS evaluation faxes

My secretary has faxed about 80 LMS evaluation forms including to 2 foreign
countries.  What we won't do for love, money and fame.

Denise Iriwn from Andver High School/Butler County Community College
(Kansas) has agreed, with some arm twisting, to put up the forms on her
website in .PDF format.  You will need Adobe Acrobat Reader (free program
at http://www.adobe.com/products/acrobat/readstep.html) to read and print
the document.  I will post the URL for her site next week when it's
complete.  I know some of the requests said they could not get faxes.
