Thanks to everyone who responded to my inquiry.  I am posting one of the
emails below which gives the information on the publication and how to
order it from ALA.

I believe you're referring to the program assessment rubric developed by
the AASL Teaching
for Learning Task Force.  The rubric, formally titled the "School Library
Media Program
Assessment Rubric for the 21st Century," is part of the 1999 AASL
publication, A PLANNING
GUIDE FOR INFORMATION POWER: BUILDING PARTNERSHIPS FOR LEARNING.  It is
available by
calling the ALA Customer Service Center at 800-545-2433, then press 7.  The
ISBN is
0-8389-8073-2 and the price is $10 ($9 if you're an ALA member).
