Program & Collection Evaluation

I would like to know what tools, assessments etc..? are being use to
evaluate middle school library programs and librarians. Please help! How are
you assessed? How is your collection assessed? Your program? How often? I
have a copy of Information Power. I'd love to see any forms/ guidelines used
at other schools. Thanks. Looking forward to your helpful replies.
