SEC: Online databases - student evaluation

We are currently previewing several online databases, using them for
specific projects with scheduled calsses.  I'd like to collect student
comments about them.  Has anybody done this?  If so, could you FAX me a
copy of your form?

I'm looking at ease of use, did it have info that helped you with the
assignment, was it easy/hard to read , etc?

If this is new territory, suggestions for questions to ask are welcome.
I'll compile them into a hit and post.
