High School Library Aide and Job Description

Hi,
I need a job description for a high school library aide and an evaluation
form.  For that matter I could also use the same for Primary, Elementary
and Middle School.  Our district does not have these in place and it really
seems to be something we need.  I did look in the archives but did not find
any.  Any help with this would be appreciated.
