Library Survey

Dear Folks,
          I need to look at some surveys any of you have created and used
to find out how the staff in your school feels about you, your library, and
your library program.  My new principal wants to allow changes in my
library program that I've wanted to try like flexible scheduling (the
school is going to block scheduling this year), but first, he wants me to
create a survey to get the "lay of the land" so to speak.  What do they
like?  What would they like to see changed? Added?  Subtracted? What would
entice them to use the library more?  Etc.  I'd really appreciate looking
at any samples to assist me in creating mine.
