Accelerated Reader Program for Grades

I am a library media specialist at Bains Elementary school.  This school
serves grades 2 - 5.  If anyone has a sample policy of how you use the
Accelerated Reader Program for receiving a grade, please send me a copy of
your guidelines.  Many teachers in our school are wanting to use Accelerated
Reader for a grade.

If anyone has sample library contracts to be signed by parents, students, and
school designee (librarian) please send them to me.

Where children are still required to sign Borrow's Cards, is it unreasonable
to expect 2nd graders to be able to sign both first and last name at the
beginning of the school year?
