needs assessment

I am taking a Collections Management course this summer and need to do a
research paper. My paper is on Needs Assessment Surveys in media centers.
This survey would be related to the entire collection and would be completed
by teachers, students, parents, and staff. If you have devised a needs
assessment survey for you media center, could you please tell me what you
included, how you administered it, and how the results were tallied and used?
