evaluating administrators

Do any of your districts use a tool for teachers to evaluate an
administrator?  Our faculty is talking about developing
an evaluation tool, and we would like to see what other districts have done.
Currently, our principals are only evaluated by the superintendent and the
superintendent is only evaluated by the school board.  If you do have
