Hello, I am interested to learn if other schools have a parental/student form or other statement of liability that must be signed in order for students to use equipment for multimedia projects. Our high school students use iBooks and digital video cameras, sometimes out of sight of classroom teachers. We have not had any serious damage, but one teacher pointed out that not all of the students are as careful as others and wondered if they might be more careful if they and their parents signed a statement of liability if the equipment is damaged (because of negligence on the part of the student). We do not want to discourage the use of the equipment, but want to avoid costly repairs and down time if equipment is out for repair. Please email me; I would be happy to post a hit. Thank you in advance for any forms or information you can provide.
