What type of circulation software do you use? We use Follett. And I know for a fact that I and the library assistant have "thought" we were checking out a book - but did not. The Follett software reminds you if a student already has the maximum number of books out or overdue books when you scan the barcode of a book to check out. You then MUST hit the "Y" that it is okay for the student to check out this book. If you don't, and hit enter....or just start with the next student, the book is not checked out to anyone. It is easy to miss hitting the "Y" at the prompt when you have lots of students lined up at the desk. I don't know if this helps you, but we have become to get very careful about this. Good luck!
