Here is the hit on laptop use.   Special thanks to Tony Doyle for letting me post 
his parental permission slip form and laptop use guidelines!



We have had laptops for check out for a couple of years now. They are actually 
starting to get outdated! We do not have a way for them to access the Internet with 
them but they are used for typing reports, preparing presentations etc. Parents are 
required to sign a form before the first time students take home a computer and I 
believe it states that they are responsible and so forth. (I don't have one in 
front of me right now.) Then each time a student wants to check one out they must 
have a teacher sing a form describing the project. Checkouts are for one night. I 
don't know of any problems we've ever had. I also seem to think that they may be 
covered under the parent's homeowner's insurance if anything would happen but I 
can't swear to that.

We check out laptops, digital cameras, video cameras, cd-burners and a few other 
things. If a laptop or other device is damaged or lost the student (parent) is 
liable for the repair or replacement cost. Checkouts are overnight only and the 
machine has to be back before first period (they are sometimes used by teachers in 
class). If a device is returned late-even if a student is sick-they lose borrowing 
privileges. I have attached copies of our rules and the parent permission form. The 
permission form was approved by our district's lawyer and the assistant supe for 
business services.

Dear parent/guardian,

In an effort to make advanced technology available to all of our students, both in 
and out of school, Livingston High School has purchased several laptop computers, a 
digital camera, and a video camera for student use. Your student has asked to be 
allowed to checkout this equipment. These items are expensive (approximate prices 
are listed below) and fragile. By giving your permission, in addition to any rights 
or remedies afforded the Merced Union High School District and/or Livingston High 
School by education code 48904 (copy attached), you agree to pay for any repair or 
replacement costs if the equipment is damaged, lost, or stolen while in your 
student's possession. You also assure that the equipment will be used in accordance 
with the following rules:

Only your student will operate or handle the equipment.

The equipment will be returned to the library before 8:00 am on the following 
school day, even if your student is absent.

The equipment will be used to complete school assignments. It may not be used for 
any commercial or political purposes.

All accessories (cables, batteries, cases, etc.) will be returned with the 
equipment (the student will be given an inventory list when the equipment is 
checked-out).

Your student will follow all applicable school and district rules while using the 
equipment, including the Acceptable Use Policy for Internet use.



If your student fails to follow these rules, he/she will no longer be allowed to 
use this equipment.

Please write your initials next to the equipment you wish to allow your student to 
use. Note: the laptop computers contain built in modems, which allow them to 
connect to other computers over telephone lines. This will allow your student to 
access the Internet through LHS's computer system. You may incur long distance 
charges if Livingston is not a local number, or if your student attempts to connect 
to some other computer or Internet service.

Laptop Computer Rules



The following rules are designed to keep the laptops in good working order so they 
are available for students when they are needed. These computers are setup to run 
on a network and violating these rules or experimenting with the machines may 
seriously affect the computers' performance.

Do not install any software on the computer. This includes plugins for Netscape.

Do not remove any programs from the machine.

Do not change any settings in the control panels or in any of the programs.

Do not install any peripheral devices (printers, scanners, joysticks, etc.).

Do not let anyone else use or handle the machine—even family members.

Do not leave the computer unguarded for even a moment.

Do not leave it in plain sight in a car (lock it in the trunk if you must leave in 
a car at all).

Do not remove the battery, disk drives, PCMCIA card, or any other device from the 
machine.

Do not experiment or play with the computer, use it only for the assignment for 
which you checked it out.

Do not try to connect to any Internet Service Provider besides LHS.

Do not attempt to alter, disable or bypass the security software.

Return the computer to the library by 7:55 am. Even if you are sick, you are 
responsible for returning the machine on time.







If any of these rules are violated you will not be allowed to checkout electronic 
equipment at LHS again.
