I showed the form to my principal, who told me that we have a family who
has the same form on file. This family got the form through their
church. I didn't ask about specific implications to the child, but
apparently the parents are taking it seriously with regard to the bio
unit. I'm not too worried about it - I always have a couple of kids here
during the bio unit anyway. Interesting form though - it rules out our
character education program (lifeskills) and other programs we have in
place to assist children who struggle with social skills, Red Ribbon
Week activities, even vision testing! Cathy
