I have always found the most successful plans are those which involve the
children themselves and which continue the philosophy and practices of the
school student management plan.  (I assume you have one of those so that
there is consistency about expectations and consequences across the school.)

I believe the library is the same as a classroom and the same standards of
behaviour apply to both, so I get the kids to agree on the sorts of things
they expect to be able to do and we write and display them.  They have input
and ownership, we use positive language and if some one is going beyond the
boundaries it is an objective way to address the issue.

For instance,
We expect to be able to work, read and study without interruption.  (So a
loud voice interrupts so I say something like "Is your voice interrupting
others?)
We expect to be able to find the resources we need in their right places (So
I ask "Is that where someone else could find that book?  Where should it be?
If you don't know, where can it be put?}
We want to have our resources clean and intact. (Have you got a library bag
to protect the book?)

You get the idea.

If a student is disruptive during a lesson then exactly the same procedures
are applied in the RC as they would be in the classroom.  I bless the day we
spent time making our whole-school plan.

All teachers at our school have been asked to submit a classroom discipline
plan.  We are a K-5 elementary school which includes special needs children.
Over the years I have used ideas from Harry Wong, Canter and others.  These
work very well in a classroom, but are not quite "right" for the library.
Would
you share some plans that have worked in your libraries?
