Here are a few things I do:

I verbally note when the book will be due when a student checks it out.

If the books were checked out by a class while they were in the library
together, I have the teacher collect the books and return them to me
when the two-week due date comes up.

For individual books, I will go to a student's class in some cases (we
don't have homeroom) and take the student to his/her locker. The book is
often right there.

I keep a cheerful attitude about books that are returned--I don't charge
fines, and I say how glad I am to have the book back, ask if the student
read it and what they thought of it, etc.

I post lists in the hallways and distribute them to the teachers (these
lists just have students' names--not titles of the books).

At the end of the year, students with any outstanding debts in my school
cannot go to the prom, do not receive diplomas, and do not receive end
of the year report cards.

Nothing very original there, but I hope some of the ideas will be
helpful.
