Tar: Form acknowledging teacher loss of materials

Hello, friends:

My principal has just asked me to create a form which lists materials a
teacher still owes the library and has a place for the teacher to sign
acknowledging the loss.  I'm really covered up today and was wondering if
anyone had such a form they would be willing to share.  Thanks so much in
advance.
