Beginning of the year questionnaire

I wanted to thank everyone who sent mural ideas.  It looks like we are going to go 
with a Florida scrub theme depicting the many animals in this area possibly with 
someone in a hammock (we live near Higlands Hammock State Park) reading a book.

On to my next project.  I am trying to come up with a questionnaire for the 
teachers.  This is what I have so far.  What other information do you think I 
should ask about?


Dear Teachers,

I am looking forward to working with you this year. Please take a moment to fill 
out the following questionnaire to help me better meet your needs. When you are 
finished you may return it to the library for a special treat. J

   What are some areas/skills you would like your students to work on in the media 
center?
   How many books would you like your students to be able to check out each week?
   What is the first unit you will be covering in science? How long do you plan to 
spend on the first unit?
   What is the first unit you will be covering in social studies? How long do you 
plan to spend on the first unit?
   What is something you liked about the activities/lessons in the media center 
last year?
   Tell about something that you felt could have been done in a better way last 
year.
   What are some specific books you would like purchased for the media center or 
areas you feel there is a need for additional books or resources?
   What other resources (videos, etc.) would you like to see added to the media 
center’s collection?
   Additional comments/suggestions:
