TARGET: Assistant Evaluation Form

I have been asked by my administration to provide a formal evaluation form
to use with my library assistant.  I have one that I used with employees at
the university level, but there are areas of work that don't apply here.

If you have evaluation forms for school library assistants that you wouldn't
mind sharing, I would love to see them.  Please email me personally.

Once my new form is complete, I will post it, if there are requests.
