Hi,

Here is my original post:
I was wondering if anyone out there had a "Library Use
Agreement". I'm not talking about a computer use
agreement. This is something that the parents would
sign saying yes, they want their children to check out
library books or no they don't.  I teach in the inner
city and I've already had several parents saying that
they didn't sign anything allowing their children to
check out.  Thanks for any help you can give me.

Here are the replies:

My school uses a pre-printed card from Demco. I don't
have my catalog at home - and there is another "big"
catalog - Highsmith! that sells them as well. It's an
index card size that has basic contact information -
name, address, phone, and a place for parents and kids
to sign agreeing to the library policies for checking
out materials. I'm not sure what these cards are
officially called - some are a gold/yellow color, and
others are a pink. These cards are attached to a
beginning of the year letter I send out with general
information - the kids see me for class once a week
and have "x" time each day to come in, costs for
replacing lost & damaged materials, encouraging
overview of programs offered, etc. Basically, the
"open house" letter for the library. We have an
automated system where I import the in coming grade
names and export the outgoing grade names. I keep
track of who has turned in cards and kids aren't
allowed to check out books until it is returned. I
usually have one or two each year that aren't allowed
to check out materials. Sometimes teachers open their
personal collections, or check out classroom books for
these students. It has worked great and never been
questioned by the parents. Check out your catalogs and
see what you can find. I keep them sorted by grade
using alphabetical tabs and plastic index card boxes -
this way, it's never questioned, and if it is, the
information is right at my fingertips!

School Library contract

Reading is fun and so is library. Checking out books
and taking them home is even more fun. But it is also
a privilege. Below are several rules and regulations
that you must read and sign and before you begin
checking out books from the library.

The number of books that each student is allowed to
check out depends on the grade they are in. 3rd and
2nd grade students are allowed 1 AR book and 1 regular
book. 1st graders start the year with just a regular
book until their teacher has approved their AR levels
for checkouts. (Unless an agreement is made between
teacher and librarian)

AR books are checked out for two weeks at a time and
may be renewed until the student is finished.

Regular books are checked out for only one week and
may be renewed two times.

When books are not returned on time there is no fine,
however the student may not checkout a book until they
return the book. If for some reason they book is
misplaced, students are given a month to find them, at
which time the book must either be returned, replaced,
or paid for (with a fee). Unless a book is damaged,
students will not be allowed to purchase their lost
book for at least a month. Overdue notices and
reminders are sent out weekly.

If you have to replace a book because of damage or
loss there is a $5.00 fee attached to the price of the
book. This fee helps with the extra cost of shipping
and handling for ordering one book at a time. However,
you do have the option to purchase a replacement copy
from one of our local bookstores, sometimes for less
than the original price. You are allowed to keep the
damaged book after you replace or purchase the damage
book. If you eventually find the book that you have
paid for, you will be refunded for the amount of the
book only. The fee is kept if the book has already
been replaced.

If you move before you have a chance to return the
book you have a couple of options. Either return the
book to our school yourself, or (if local) at the new
school you are attending to return it to our school.
Your transcripts will not be forwarded to the new
school until your books are returned.

By signing this contract you and your parent/guardian
agree to the rules and regulations for checking out
books from the Terrace Heights Elementary Library.

     Other people said that they sent home letters at
the beginning of the year outlining library
procedures.
     I'll probably come up with a letter similar to
the one above to use with my students.  Thanks for
your help.
