just post what I received.  Hope this helps you.

Hi Wendy,
I have been on spring break so I just saw your post. We have to input our
lesson plans in the district lesson plan computer system so I really do not
have one I can attach to you.

I did the lesson pretty much as I described it to you.  You will need to
make a copy of the map in the center of the book.  I made multiple copies
so
each student could hold one while I read the story.  They followed the
progress of the story on the map.  I was able to stop at certain points and
ask them, about directions and landmarks. You will probably find yourself
improvising all kinds of questions that require map skills to answer.  The
Dish ran away with the spoon also became a lesson on Allusions.  The kids
had fun picking out the allusions as we read the story.

The most time consuming part of the lesson was making the maps.  You could
also make one map on an overhead and track the story that way or blow the
map up on a poster.  The kids also had fun locating the fork hiding in each
picture.
