TARGET: Elementary student helpers

Does anyone have the following: ?

1. an application for students who are interested in becoming a student library 
helper whereby the interested student must get a teacher's recommendation: from a 
math teacher to show potential competence in decimals (shelving nonfiction) and 
from an English/language arts teacher to show   competence in alphabetical order 
(easy, biography, and fiction).
2. a written shelf order test.
