I would like to have my 4-5-6th graders fill out an end-of-the-year questionaire regarding the library and I was wondering if anyone had a form for me to get ideas from so I don't have to start from scratch. I was thinking of questions such as: 1. What was your favorite lesson? 2. What did you wish you had learned that you didn't? 3. Do you feel comfortable using the following reference items: (computer (library's automation program), Internet, encyclopedia, almanac, atlas, dictionary, thesaurus)? 4. What was your least favorite lesson? 5. Any suggestions for next year? 6. What type of books did you enjoy reading? 7. Would you have liked more booktalks? 8. Suggestions for books to order for next year? Anything I'm missing. Thank you.
