I must have missed the original post, but I want to recommend the Bluford Series,
available directly from Townsend Press on their website:

Written at about a 4th grade level, the books depict the life of students at an
urban high school called Bluford High.
Townsend sells these books for $1.00 each--that's right, folks, a buck a book!!!!!!!
And there is a teacher guide too. Kids LOVE these books. I can't keep them on the
shelves. The topics of the books range from bullying, to dating violence, family
break-up, etc. They are "realistic" novels.
I have had parents come in to tell me that those books have turned their
non-reading children into avid readers. Low reading ability kids have gone up two
grade levels in a year just because these books opened the door to them. After
they read the series, I then lead them to books by Walter Dean Myers, Jacqueline
Woodson, Sharon Flake, Alex Flynn, etc. I've seen several "graduate" to adult books.
