[LM_NET] TAR: student surveys

Hello-
    I have searched the archives for student surveys and have not found quite what 
I want.  I am currently working on my master's degree and need to an action 
research project.  I will teaching the Big 6 research method with a final result of 
a PowerPoint to a class of 5th graders.  What I need is some examples of surveys 
students would fill out about how they feel about the library and if they feel the 
library helped them with the research.  Does anyone have anything similiar they 
would be willing to share?  Please e-mail me off list.  Thanks in advance for all 
your help.
