Tried to be more specific in the subject line, but can't figure out what to call
what I need. My principal has asked me to design a form that staff can easily fill
out, that would record all the details of a specific event at our school. The idea
is to make it easy to compile a historical record of our school. It doesn't have
to be a major event like a play or sports championship. It could be some details
about the day we had to evacuate the building because of a gas leak -- that kind of
thing. We are in our seventh year of operation and the library has been saving
programs, newspaper articles, yearbooks, etc., but my principal is afraid that many
events will not be remembered if we don't do something like this form. Have any of
you already created something like this that you would be willing to share???? I'd
be most grateful.

Thanks in advance.
