[LM_NET] GEN: MLA citation help / advice needed

How would you list the author for this book using MLA
format?  (It has me stumped.)  Here is the
information:  

LOC CIP DATA
Lee, Carol Ann.
A friend called Ann / by Jacqueline van Maarsen;
retold for children by Carol Ann Lee.

I've had the student list the author as Carol Ann Lee,
but I don't know where to put "Jacqueline van Maarsen"
or if it should be included at all.  (We subscribe to 
NoodleTools to put our reference lists / works cited
pages together.)
