Thanks to everyone for their interest and replies to this topic. This sounds like something that many have thought about doing. Unfortunately the only concrete recommendations I received were the ones below. I was able to piece together a survey from several others. I just tried to decide what questions I wanted answered; what I felt was most important. My teachers who have mostly seniors are going to distribute them for me. If anyone would like to see the finished product just let me know and I'll send you a copy. Thanks so much for your help!! ******************************************************************************************** We used to send a survey home in their invitation to come to the library for the "Senior Tea." This event is sponsored by the Guidance Dept to get the graduated seniors back in school around Christmas time when they would be home for break. Current seniors are invited to attend, mingle and learn about college life, (or work, or military). It has been a big hit and most everyone comes to see each other and make plans. Unfortunately, they rarely remember to bring the survey! It's a lost cause to ask them to fill it out during the Tea. So, our new idea is to ask students to sign in at the tea and give their e-mail address. I think it will be more efficient. We get great responses which we use in brochures and orientations. The form includes: NC LIBRARY GRADUATING SENIOR SCHOLARSHIP AWARD Seniors: We have thoroughly enjoyed working with you during your high school years. We hope to recognize one of our own library users. Based upon your responses to the items below, a maximum of three students will be selected by a scholarship selection committee. Please submit your responses in an envelope that also indicates your name, address and phone number to the NCHS Library by May 11th. 1. Provide a brief statement of how the NCHS Library and its staff have helped you in your career as a student here at NCHS. 2. What, about the NCHS library, is most memorable for you? 3. Provide suggestions about how the NCHS Library can improve its services for the students. 4. List titles of books you recommend for the students following in your footsteps. If you permit us to use your name and cite your responses in our future brochures, publications, and presentations, please sign and date this form. Thanks and good luck.
