These are the responses I received. Thank you to all who responded.

1. Look again at Amazon:

http://www.amazon.com/Joy-Signing-Illustrated-Mastering-Language/dp/08824352
05

-- available new and used.

My recommendation would be to find a copy of that book, by interlibrary loan
if necessary, or a used copy if not new, and go directly to the source. If
citing a (copyright-infringing?) reproduction of portions, then I would
recommend citing it roughly as follows:

Lottie L. Riekehof, The Joy of Signing [and publication data], as quoted in
[and here cite author, title, and any other info. about the booklet actually
used as the student's source].

2. I own a copy of Joy of Signing. My copy would be MLA cited thus:

Riekehof, Lottie. The Joy of Signing, 2nd ed, Springfield, MO: Gospel
Publishing House. 1987.

I think, though, that citing the Jones booklet would be sufficient, and that
Jones is citing her source. That would be:

Jones, Karyn. An Introduction to Sign Language. N.p. N.d.

The N.p. and N.d. stand for "No place [of publication available]. No date
[of publication available].

3. cite the book that was used. Since the student used the spiral bound
examples, and not the original book, it would be inaccurate to cite the
original book
