[LM_NET] ELEM: target: note taking sheet

Greetings,
I am starting a 3rd grade polar animal research project tomorrow.  The
end product is going to be a non-fiction "book."  I would love to have a
new note taking sheet for the students.  Does anyone have a
tried-and-true sheet that "works every time" that they would like to
share with me?
