[LM_NET] TAR: parent checkout contract?

I am speaking to a parent group tonight, letting them know what services the media 
center offers for them and their children.  We let parents sign up to checkout 
books to read with their children and have an index card we keep with their contact 
information.   Do any of you have something you have the parents sign prior to 
checking out books?  A contract of sorts stating the responsibilities of checking 
out school library books?
