[LM_NET] Copyright: Do You Use a Standard Permissions Form?

Good Afternoon,

I am looking for a school district "Standard Permissions Form" for requesting to 
use copyrighted materials such as music, video, print, graphics etc. in a K-12 
setting (that would not be covered by Fair Use). I have Dr. Carol Simpson's book 
and see the sample on page 160 but was wondering if there are forms out their that 
school districts use that may be different than this example.

If you have access to your form, I would be interested in viewing it. Or if you use 
the sample in Dr. Carol Simpson's book, that would be nice to know.
