[LM_NET] TARGET: Elem librarians

Good morning!
I am an elem librarian with a fixed schedule.  I would like to collaborate
more or at least, be able to support the teachers with their curriculum with
my library lessons next year.  I think I do a good job of that now, but need
input from the teachers.  I often think they are studying a topic (based on
county pacing guide) but then they aren't due to snow, field trips,
rescheduling, etc....

My idea is to have a form that I will give team leaders once a month. The
teachers will fill it out with library materials they need for the next
month, topics they will be studying, etc...along with a question that will
state How can I help you.

If anyone has a form that I can look at (I won't copy it) I would love to
see it. Please send to me as an attachment.  Thank you so much.
