We are discussing forms over our local Washington k-12 list-serv and
thought I'd post this to everyone in answer to Ruie's request...

Madelynne,
        This form was compiled by Marge Coy at ESD 112.  Marge said she
used many resources, including the net, when creating it.  She wanted to
make sure you knew she read and followed the Wednet guildlines.  Marge
also advised that you adopt one release form for the entire school
district.  It would eleminate many problems and inconsistencies.  The
approval must come from the superintendent.

                 Acceptable use Policies for Internet Access

1.  All use of the Internet must be in support of education and research
and consistent with the purposes of____________________________ School
District, ESD 112, and WEdNet.

2.  Any use of the network to facilitate illegal activity is prohibited.

3.  Any use of the network for commericial or for-profit purposes is
prohibited.

4.  Extensive use of the network for non-work related communication is
prohibited.

5.  Any use of the network for product advertisement or political
lobbying is prohibited.

6.  Network accounts are to be used only  by the authorized owner of the
account for the authorized purpose.

7.  Users shall not intentionally seek information on, obtain copies of,
or modify files, other data, or passwords belonging to other users, or
misrepresent other users on the network.

8.  All communications and information accessible via the network should
be assumed to be private property.

9.  No use of the network shall serve to disrupt the use of the network
by others; hardware or software shall not be destroyed, modified, or
abused in any way.

10.  Malicious use of the network to develop programs that harass other
users or infiltrate a computer computing system and/or damage the
software components of a computer or computing system is prohibited.

11.  Hate mail, harassment, discriminatory remarks, and other antisocial
behaviors are prohibited.

12.  The illegal installation of copyrighted software for use on district
computers is prohibited.

13.  Use of the network to access obscene or pornographic material is
prohibited.

14.  Use of the network to transmit material likely to be offensive or
objectionable to recipients is prohibited.

15.  Subscriptions to Listservs must be pre-approved by the building
Internet coordinator.

16.  Diligent effort must be made to delete mail daily from the personal
mail directory to avoid use of fileserver hard disk space.

>From time to time, _______________________ School Disrtict, ESD 112, and
WEdNet will make determinations on whether specific uses of the network
are consistent with the acceptable use practice.
__________________ School District and ESD 112 reserve the right to log
Internet use and to monitor fileserver space utilization by users while
respecting the privacy of user accounts.  ______________________ School
District and ESD 112 reserve the right to remove a user account on the
network to prevent further unauthorized activity.  WEdNet and ESD 112
reserve the right to disconnect an Affiliate (School District) to prevent
further unauthorized activity.
