TARGET: ELEM & MIDDLE: Evaluations and goal setting

Colleagues--

Have any of you developed evaluations of the media center that you ask
staff and/or students to fill out and then use for goal setting?  I have
seen references to these and would like to see some before starting from
scratch on my own.  Would any of you be willing to share?
