Target --> Computer Repair Forms

Hi everybody!

As the summer is winding down (UGH) and school starts soon, I need to make a
form to help me manage computer repairs (yes, it's one of my many jobs).  I'm
thinking of something which would ask questions like "When I try to
______(print, etc.), I get this message on the screen ____________.  This
would include info like the program running, user names, and priority to
continue with class activities.  (This is so I know if the final exam which
starts tomorrow is on the computer, etc.)

Does anybody out there have sample forms they could mail or fax?  Questions
from their forms that they could email?  Suggestions for things I should add?


I'll post a hit in about a week and a half.  I'm off to the beach before I'm
back to work officially.
