Target: Looking for elementary title

I had a former student of our elementary school call today looking for the
title of a story that was read to her class about 15 years ago and I
thought maybe someone who has been in the field a little longer than me
might know what the story is.

It is about a girl who goes around wearing a green ribbon around her neck
all of the time and it ends with her taking the ribbon off and her head
falls off.  Any help would be appreciated.  TIA.
