Evaluation Form Requests

Dear Group:

I had no idea how many people would want a sample librarian evaluation form
when I mentioned it last week...over thirty and still counting.  To
facilitate things and save a bit on postage, I will put it in e-mail form
and send it to those who requested it.  I probably won't have time to do
this until Thursday of Friday of this week (we are off those days) but I
promise to do so for all who asked.

It may not be as usable as the original but I'll do my best to make it so.

Thanks for your interest...you're a great group.
