Well it doesn't look like a school generated form and I doubt any school
would accept it.  It should probably be sent straight to the district
office and the district's lawyer.  They could probably come up with better
wording and come to a clearer understanding with the parents about what
they want.

Some of the things listed are vague or subjective terms.  For example
"globalism", "multiculturalism", "alternative lifestyle", "environmentalism
that is biased", and "affective education" could all be interpreted very
differently depending on your political bent.  Is reading books by Walter
Dean Myers or Gary Soto multiculturalism?  Is studying the United Nations
or NAFTA promoting globalization? Are multiracial families living an
alternative lifestyle?  And does the parent know that all of these taboo
subjects are covered in the library collection?

"3.  Request that there be no referral for any prevention, intervention or
after care program or services of any organization or agency (whether
internal or external), even if the law might allow treatment without
parent/ guardian consent."
--The school probably could not legally agree to this since it might
prevent the reporting of suspected child abuse by a parent

I just received this "Opt out Form" from a parent.  I was wondering if
